瘦素与端粒酶对良恶性胸腔积液的诊断价值

摘    要:目的探讨胸腔积液瘦素浓度、端粒酶活性对良恶性胸腔积液的诊断价值。方法检测29例恶性胸腔积液患者胸液中的瘦素浓度和端粒酶活性,以28例良性胸腔积液作对照。结果恶性胸腔积液中的瘦素浓度较良性者增高,差异有统计学意义(P<0.05);瘦素的敏感性82.76%,特异性92.86%,准确率87.72%;端粒酶的敏感性89.66%,特异性82.14%,准确率85.96%。结论瘦素与端粒酶对恶性胸腔积液的诊断与鉴别诊断有一定的临床价值,联合检测瘦素与端粒酶对恶性胸腔积液诊断具有较高的诊断价值。

Abstract:Objective To investigate the clinical value of leptin and telomerase in diagnosis of malignant pleural effusion. Methods Twenty-nine specimens of malignant pleural effusion and twenty-eight specimens of nonmalignant pleural effusion were examined for leptin concentration & telomerase activity in pleural effusion. Results The concentration of leptin and telomerase in malignant pleural effusion were significantly higher than those in benign pleural effusion. The diagnostic efficiency of leptin in pleural effusion as following: sensitivity 82.76%, specificity 92.86%, accuracy 87.72%. The diagnostic efficiency of telomerase activity in pleural effusion as following:sensitivity 89.66% ,specificity 82.14% ,accuracy 85.96%. Conclusion Leptin and telomerase activity have some diagnostic value in differentiating benign from malignant pleural effusion. Diagnostic rates will be elevated by joint detection of leptin and telomerase activity pleural effusion .
