摘    要:目的:通过动物实验验证肝脾固定治疗小儿肝前性门脉高压的可能性。方法:将犬随机分为实验组(n=10)和对照组(n=5),分别制造肝前性门脉高压模型,然后对照组关腹,实验组将肝脾加以固定后关腹,饲养12周后以彩超检查肝脾问血流情况,开腹测脾脏大小、门脉压力,抽取血样,经脾穿刺,行X线造影,并取活组织进行病理学检查。结果:2组动物模型的制作均获得成功,压力变化、病理结果、X线检查和彩超检查均证实实验组肝脾固定良好,能够将门脉血液分流回肝,降低门静脉压力。结论:肝脾固定后能有效降低门脉压力.治疗门脉高压具有可行性.并且更符合生理状态。

Abstract:Aim: To validate the possibility of curing pre hepatic portal hypertension in children by fixation of liver with spleen. Methods: Fifteen dogs were used to make pre hepatic portal hypertension models by partially ligating the portal vein so that portal vein pressure increased about 3 times higher than pre operation.Six dogs as control group, in which abdomen cavity was closed.Nine dogs as experiment group,in which the liver and spleen was sutured and then the abdomen cavity was closed.Blood stream through the sutured area was detected by color ultrasound 12 weeks later. Other programs such as portal vein pressure, cicatrisation between liver and spleen, and liver biopsy were also done. Results: Ultrasound and contrast examination showed that the blood stream could come and go between live and spleen; pressure measure showed that it could effectively decreased portal vein pressure; pathologic observation revealed satisfactory cicatrisation between live and spleen parenchyma. Conclusion: The results showed that the method can effectively decrease portal vein pressure, and is feasible for curing pre hepatic portal hypertension.
