抗环瓜氨酸肽抗体在类风湿性关节炎诊断中的应用评价

目的探索抗环瓜氨酸肽(CCP)抗体在类风湿性关节炎(RA)病人诊断中的应用价值。方法检测了124例血清抗CCP抗体和IgM-RF,其中52例确诊RA患者,29例临床怀疑但未确诊RA患者,43例确诊非RA风湿性疾病患者,并结合患者临床资料进行分析。结果(1)抗CCP抗体诊断RA的敏感性为63.5%,特异性为97.67%,IgMRF敏感性为73.08%,特异性为67.44%,二者对RA的特异性差异有统计学意义(P<0.05);(2)临床怀疑但未确诊RA患者中抗CCP抗体阳性者经2年随访有77.78%确诊RA,显著高于抗CCP抗体阴性者。结论(1)抗CCP抗体对RA的诊断有高特异性;(2)抗CCP抗体阳性者有发展为RA的高危险性。

An evaluation of the Diagnostic Value of Anti-cyclic Citrulinated Peptide Antibodies in Rheumatoid Arthritis Disease

Objective To estimating the diagnostic value of anti-cyclic citrullinated peptide (anti-CCP) antibodies in rheumatoid arthriti s(RA) disease. Methods 52 RA, 29 suspected RA and 43 rheumatism were test ed.The sera were detected for anti-CCP antibody and lgM-RF.The clinical master ies were studied. Results 1.The sensitivity and specificity of anti-CCP fo r RA were 63.5% and 97.67% respectively,while it were 73.08% and 67.44% in Ig M-RF.The difference of specificity for RA had statistical significance(P<0 .05);2.Among the suspected RA,77.78% of those who were positive with anti-C CP 2 years later. Conclusion 1.Anti-CCP has a high diagnostic specificity for R A;2.Those who have positive anti-CCP may run a high risk in developing i nto RA.
