腔镜下等离子双极电汽化术治疗臀肌挛缩症

目的探讨腔镜下等离子双极电汽化治疗臀肌挛缩症的可行性及临床效果.方法本组自2005年6月～2006年1月对14例年龄5-20岁患单/双侧臀肌挛缩症病人通过大转子上方建立人工腔道,以生理盐水为灌洗液介质,使用等离子双极电汽化术进行挛缩束带的刨削、切断,达到改善髋关节的活动范围,术后2天下床行早期功能锻炼.结果术后随访3个月～半年,平均4个月,根据黄耀添等疗效评价标准进行综合评价,优12例,良2例.全部伤口一期愈合,无血肿形成、无神经血管损伤.结论腔镜下等离子双极电汽化松解臀肌挛缩束带具有操作简便、微创安全、无出血、痛苦小、疗效可靠、住院时间短、等优点,值得推广使用.

Treatment of the gluteal muscle contracture with plasma bipolar electrovaporization resection

Objective To explore the feasibility and the clinical effect of the treatment of the gluteal muscle contracture with plasma bipolar electrovaporization resection under the endoscope. Methods From July 2005 to January 2006,14 patients unilateral/bilateral gluteal muscle contracture were treated.The artificial cavities were established through superior part of greater trochanter and physiological saline was used as the cavity lavage solusion.Plasma bipolar electrovaporization technique was applied.Shaping,resecting of contracture ties and belts were performed in order to improve the movement range of hip joints. Early exercise were encouraged since the 2nd days after operation. Results All the patients were followed-up for 3-6 months. According to Huang Yaotian's eurative effect cvaluation criteria,the excellent effect was got in 12 cases,and good effect in 2 cases.All wounds healing were first intention,without hematoma and blood vessels and nerve damage. Conclusion This operative procedure has many advantages, such as simple manipulation,reliable efficacy,minimally invasive,safe less painful and a shorter hosipitalization etc.
