Graves病合并多系统脏器功能损害的131I治疗临床分析

目的探讨Graves病并发多器官损害的临床表现及131I治疗转归,提高临床诊治水平。方法回顾性分析47例Graves病并发多器官损害患者的临床资料,对临床诊断和治疗进行分析。结果 131I治疗后6个月复查,甲亢治愈42例(89.36%),其中甲低11例(23.40%);5例(10.64%)患者缓解,其中2例(4.26%)继续观察,3例(6.38%)进行二次治疗。甲亢治愈患者肝功、彩超及心电图恢复正常,白细胞总数明显上升。未见发生粒细胞缺乏患者,无发生甲状腺危象者。结论 131I治疗甲亢安全有效,对于甲亢伴发多系统脏器功能损害者应作为首选治疗。

The Clinical Analysis of 131I Treatment of 47 Cases of Graves Disease Complicated by Multiple Organ Damage

Objective To investigate the clinical symptoms of Graves disease complicated with multiple organ damage and the prognosis after 131I treatment so as to improve the level of diagnosis and treatment of the disease. Methods The data of clinical diagnosis and treatment of 47 Graves disease patients with multiple organ damage were analyzed retrospectively. Results During the foUow-up 6 months after 131I treatment, 42 cases were cured （89.36%）,including 11 cases of hypothyroidism （23.40%）. 5 cases were partial remission （10.64%）, of which, 2 cases continued to be observed （4.26%）and 3 cases were given the second course of treatment （6.38%）. The liver functions, the results of color Doppler ultrasound and electrocardiogram of the hyperthy- roidism patients returned to normal and the total number of white blood ceils of them increased significantly. No patients with neu- tropenia or thyroid crisis were found. Conclusion 131I therapy is safe and effective for the treatment of hyperthyroidism with mul- tiple organ damage, which should be the first choice for the treatment of the disease.
