摘    要:目的 观察脑电双频指数(BIS)监测经喉罩吸入七氟醚在小儿短小手术中的应用效果.方法 择期短小手术患儿30例,诱导时吸入8%七氟醚+ 5 L·min-1氧,BIS降至45~50插入喉罩,术中吸入七氟醚1.5%~3%+ 2 L·min-1氧,控制BIS 40~60,保留自主呼吸,必要时进行手控辅助呼吸.缝皮结束,停止吸入...

Abstract:Aim To investigate the effects of sevoflurane inhalation anesthesia with laryngeal mask in pediatric minor surgery while bispectral index monitoring.Methods Thirty children were scheduled for minor operation.Anaesthesia was induced with 8% sevoflurane and 5L·min-1 oxygen,and maintained with 1.5%~3% sevoflurane and oxygen 2L·min-1.Laryngeal mask was inserted when bispectral index(BIS)at 45~50.Spontaneously breathing was reserved and BIS was maintained at 40~60 during operation.Sevoflurane inhalation was closed when operation was completed and laryngeal mask was extracted when BIS was about 75.HR,SBP,SpO2,RR and PETCO2 were recorded at laryngeal mask insertion,skin incision,intraoperation,skin suture and laryngeal mask extraction.Induction time,palinesthesia time,the airway reactivity during induction and postoperative restlessness were also recorded.Results SBP,HR and RR were significantly higher than those before anesthesia only at laryngeal mask extraction(P0.05),the induction time and palinesthesia time were(123±33)s and(15±2)min respectively.The end-tidal sevoflurance concentration at laryngeal mask insertion was(4.2±0.5)%,(2.1±0.4)% at maintenance of anesthesia,and(0.5±0.1)% at laryngeal mask extraction.No child had the airway reactivity and postoperative restlessness.Conclusion Monitored by bispectral index,sevoflurane inhalation anesthesia with laryngeal mask has quicker induction and palinesthesia,steadier haemodynamics during the operation,and better controllability.
