老年肺结核的临床特点分析

目的对老年肺结核的临床特点进行分析和探讨,以提高诊治水平.方法对近5年来老年肺结核的30例病例的诊疗情况进行回顾性分析.结果老年肺结核患者病史和肺部钙化点者16例,以咳嗽、喀痰为主要症状者24例,9例并发呼吸道感染就诊,4例无症状.痰菌阳性8例.肺部病变以右上肺为主.治愈26例,迁延不愈3例,死亡1例.结论老年肺结核的临床特点不典型,合并症多,免疫力低,延误治疗率高,复发率高,治疗难度大.因而要注重在临床诊断上的综合性分析和在治疗上的个体化.

Clinical characteristics in elderly patients with pulmonary tuberculosis

Objective: To analyze the characteristics of pulmonary tuberculosis (TB) so as to improve its diagnosis and therapy. Methods: 30 cases of TB in the last 5 years(1 female and 29 males), aged 60~91 (mean age 76), were included in our studies. Acid fast stain of sputum, bronchoscope and pathologic exam, purified protein derivative (PPD) study were performed in 21 cases and erythrocyte sedimentation rate tests were performed in all patients. Antituberculosis treat was done. Results: 16 cases were found with pulmonary calcification and fibrosis, 4 cases with no clinical symptoms, 8 cases with positive acid-fast stained sputum. Most lesions were in the right supper lobe. 26 cases got cured. 3 cases had a protracted course and 1 died. Conclusions: The clinical symptoms of elderly patients with TB were non-specific, being complicated with various illnesses and poor toleration in chemotherapy. So comprehensive analysis of the clinical characteristics is important and the treatment plan should be taken on the basis of personal feature.
