慢性阻塞性肺疾病患者血浆甲状腺激素水平的临床观察

目的:探讨慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者血浆甲状腺素水平的含量变化及其临床意义。方法:选择40例COPD急性加重期及稳定期患者为研究对象,35例年龄匹配健康体检者为对照组。检测3组血浆甲状腺激素(TT3、FT3、FT4、TT4)及超敏促甲状腺激素(hTSH)水平。同时检测COPD急性加重期患者动脉血氧分压水平,并分析其与PaO2的相关性。结果:COPD患者急性加重期及稳定期血清TT3、FT3、FT4水平显著低于健康对照组(P<0.05)。COPD急性加重期血浆TT3、FT3、FT4水平显著低于稳定期(P<0.05)。COPD患者急性加重期PaO2与血浆TT3、FT3、FT4水平呈正相关。结论:COPD急性加重期血浆TT3、FT3、FT4水平显著低于同龄健康对照组,血浆甲状腺素水平测定及动态观察有利于COPD病情严重程度的判断及预后估计。

Study of altered thyroid hormone level in patients with chronic obstructive pulmonary disease

Objective: To investigate the change and clinical significance of thyroid hormone level in patients with chronic obstructive pulmonary disease( COPD). Methods: Serum total triiodothyronine( TT3),total thyroxine( TT4),thyroid-stimulating hormone( TSH),free triiodothyronine( FT3),and free thyroxine( FT4) levels were analyzed by electro chemiluminescence in 40 patients with COPD,either in the acute exacerbation phrase or stable phrase,and in 35 age-matched healthy subjects. At the same time,the blood gas analysis of arterial blood was also determined. Results: The levels of TT3、FT3、FT4in patients with COPD were much lower than that in the controls( P < 0. 05),and the acute exacerbation phrase was much lower than the stable phrase( P < 0. 05). Besides,serum TT3、FT3、FT4levels were positively correlated with PaO2. Conclusion: Compared with age-matched healthy controls,serum TT3、FT3、FT4levels in patients with acute exacerbation COPD were significantly lower,which might be valuable for determining the condition and predicting the prognosis.
