| 摘 要： | 目的:探讨腰麻-硬膜外麻醉在老年患者髋关节置换术中的临床疗效及安全性评价。方法:对2009年1月-2011年4月我科30例行髋关节置换术的老年患者,使用腰麻联合硬膜外麻醉,监测麻醉起效时间、最高感觉阻滞平面,麻醉前、麻醉药后1min、10min、30min时的收缩压/舒张压(SBP/DBP)、经皮血氧饱和度(SpO2)、心率(HR)、呼吸(RR)、运动神经阻滞情况(Bromage氏评分,BMS)和术后疼痛(VAS评分),评价麻醉效果、对患者血流动力学影响、术后疼痛情况。结果:30例患者均成功结束手术,麻醉起效时间平均为(80±19)s,阻滞范围为T10～S5,麻醉后10min SBP/DBP有下降,与麻醉前比较,差异有统计学意义(P<0.05),麻醉前后SpO2均>95%;BMS、VAS评分分别为(3.5±0.5)分、(15±6)分。结论:腰-硬联合麻醉起效快,对循环波动相对较小,可安全用于老年人髋关节置换术的麻醉。 |
