摘    要:目的:探讨显微神经外科技术在切除鞍区垂体瘤和颅咽管瘤手术中的临床应用价值.方法:应用显微神经外科技术切除17例垂体瘤、4例颅咽管瘤,分析临床疗效和手术并发症.结果:垂体瘤全切12例,次全切除5例;颅咽管瘤全切1例,次全切除2例,大部切除1例.无严重手术并发症和死亡病例.结论:熟悉鞍区显微解剖结构,合理应用显微神经外科技术,是提高鞍区肿瘤切除率、降低手术并发症的关键.

Abstract:Objective:To explore the value of applying microneurosurgical technology to remove craniopharyngioma and pituitary tumors in saddle area.Methods:17 cases of pituitary tumors and 4 craniopharyngiomas received tumorectomy with help of micro-neurosurgical technology.The clinical effect and postoperative complication had been evaluated and analyzed.Results:Complete excision of pituitary tumors were performed on 12 subjects,partial excision on 5.Meanwhile,one patient with craniopharyngioma accept complete excision,2 received subtotal resections,and another patient had major resection.No serious complication and death occurred to these patients.Conclusion:In order to increase the achievement ratio of sellar tumorectomy and decrease postoperative complication,it is key point to be familiar with the microanatomy in saddle area and employ micro-neurosurgical technology reasonably.
