胃原发性恶性淋巴癌10例临床分析

目的　通过观察胃原发性恶性淋巴瘤的临床 ,胃镜、X线表现及治疗结果 ,探讨提高早期诊断率。方法　复习近年来经手术和病理确诊的 10例胃原发性恶性淋巴瘤进行分析。结果　 2例经胃镜活检病理证实 ,8例均手术病理证实 ,随访病例 2例于手术后一年死亡。结论　胃原发性恶性淋巴瘤的临床表现无特异性 ,胃镜检查必须探挖多取才能得到阳性结果 ,结合 X线及活检组织免疫组化检查可提高早期诊断率。治疗以手术后化疗 (CHOP方案 )为主 ,清除 HP是治疗的关键 ,其预后与临床分期有关

A CLINICAL ANALYSIS OF 10 CASES OF PRIMARY GASTRIC MALIGNANT LYMPHOMA

Objective To try to improve early diagnosis of primary gastric malignant lymphoma. Methods Analyse the clinical manifestations. radiologic and endoscopic findings in 10 cases. Results 2 patients were diangosed by means of endoscopic biopsies while the other 8 ases were proved by operation, among them 2 patients died one year after operation Conclusion No specific clinicil manifestations were found. Large biopsy specimens and multiple biopsy specimens of any suspicious area are essential for early diagnosis. The radiographic and endoscopic finding are described and immunohistological examinations are helpful. CHOP regieme is the choice of chemotherapy. Erradiation of H. pylori is important. The prognosis is related to the clinical staging.
