心电图诊断左室肥厚的再评价

本文通过对１９３例高血压病患者住院病历的分析，发现（１）左室心肌质量指数高于正常者（男＞１３４ｇ／ｍ２、女＞１１０ｇ／ｍ２）共９０例，其心电图左室肥厚的敏感度为１８．９％，特异度９３．３％。当左室心肌质量指数男＞１７４ｇ／ｍ２、女＞１５０ｇ／ｍ２时，心电图左室肥厚的敏感度最高，为３３．３３％，特异度９０．９％。但心电图左室肥厚的敏感度并非随左室心肌质量指数增加而增加。（２）心电图左室肥厚组与正常组比较其左室心肌质量指数有显著差异，Ｐ＜０．０１。肥厚组与高电压组的心肌质量指数比较Ｐ＞０．０５。（３）年龄、性别对心电图左室肥厚的敏感度无影响。

Reassessment of Electrocardiographic Diagnosis in Left Ventricular Hypertrophy

hypertension patients were studied.The following points were found (1) 90 of the 193 cases revealed higher echo left ventricular mass index(LVMI) than normal,namely,>134g/m2 in man and > 110 g/m2 in woman. For these criterions the sensitivity and specificity of ECG in diagnosing left ventricular hypertrophy(LVH) was 18.9% and 93. 3% respectively.When LVMI > 174 g/m2 in man and > 150 g/m2 in woman,the sensitivity of ECG was the highest (33. 33% ).(2) An obviously significant difference of LVMI was shown between the ECG - displayed LVH group and non - LVH group, p<0.01.There was no significance (p>0.05) in LVMI between LVH group and high voltage group in ECG.(3) Age and sex were not attributed to sensitivity of ECG.
