中药外敷联合骨盆带治疗产后耻骨联合分离及对生活质量的影响

目的研究中药外敷联合骨盆带治疗产后耻骨联合分离的效果,以及对患者生活质量的影响。方法选择2014年6月-2017年2月期间收治的88例产后耻骨联合分离患者作为研究对象,按照随机数字表法,将其分为对照组和观察组各44例。所有患者均采用产后常规护理,对照组采用骨盆带治疗,观察组采用骨盆带联合中药外敷治疗,连续治疗1周。比较两组患者治疗前后耻骨联合分离宽度、疼痛程度(VAS)评分、生活质量评分、临床疗效以及不良反应发生情况。结果治疗后两组患者耻骨联合宽度均减小,且观察组低于对照组(P<0.05),差异有统计学意义。治疗后第1、3、7天,观察组患者VAS评分较本组治疗前均明显下降,且低于对照组;对照组患者在治疗后第3、7天,VAS评分较本组治疗前明显下降(P<0.05),差异有统计学意义。治疗后,两组患者日常活动、睡眠、食欲、精神状态和社会功能评分均较本组治疗前升高,其中观察组各评分均高于对照组(P<0.05),差异有统计学意义。观察组治疗后有效率高于对照组(P<0.05),两组患者治疗期内未出现明显不良反应。结论对产后耻骨联合分离患者采用骨盆带联合中药外敷治疗,效果优于单用骨盆带,且能够快速缓解患者疼痛,改善生活质量,无明显不良反应,值得临床推广。

Effect on postnatal separation of symphysis pubis and the quality of life in treatment with the external application of Chinese herbal medicine and pelvic girdle

Objective To study the effect on postnatal separation of symphysis pubis and the quality of life in treatment with the external application of Chinese herbal medicine and pelvic girdle.Methods From June 2014 to February 2017,88 patients with postnatal separation of symphysis pubis were collected as the subjects.According to the random number table,they were divided into a control group and an observation group,44 cases in each one.The routine postnatal nursing was provided in the patients of both groups.Additionally,the pelvic girdle was used in the control group and the the combined therapy with pelvic girdle and the external application of Chinese herbal medicine was given in the observation.The treatment was provided consecutively for 1 week.Before and after treatment,the symphsis pubis separation width,pain degree(VAS score),the score of quality of life(QOL),clinical effect and adverse reaction were compared before and after treatment in the two groups.Results After treatment,the width of symphysis pubis was reduced in the two groups and the width in the observation group was lower than the control group(P<0.05),indicating the significant difference.In 1,3 and 7 days after treatment,VAS scores in the observation group were reduced obviously as compared with before treatment and they were lower than the control group.In the control group,in 3 and 7 days of treatment,VAS scores were reduced obviously,indicating the statistical significance(P<0.05).After treatment,the scores of daily activity,sleep,appetite,mental state and social function were all increased and the scores in the observation group were higher than the control group,indicating the statistical significance(P<0.05).The effective rate in the control group was lower than the observation group(P<0.05).During treatment,no obvious adverse reaction occurred in the two groups.Conclusion The combined treatment with pelvic girdle and the external application of Chinese herbal medicine achieves the better effect on postnatal separation of symphysis pubis as compared with simple use of pelvic girdle.This combined therapy rapidly relieves pain,improves the quality of life and has no adverse reaction in the patients,hence,it deserves to be promoted in clinical practice.
