CT灌注成像肝细胞癌边缘血流灌注与肿瘤转移和预后的关系

目的 采用CT灌注成像研究肝细胞癌边缘血流动力学变化和肿瘤发生转移的关系,以探讨预测肝癌发生转移的潜在可能性.方法 选择34例经手术病理证实的肝细胞癌患者,术前全部病例进行多层螺旋CT灌注成像,统计瘤周组织的血流灌注指标,并将结果与患者随访情况进行比较分析.结果 ①肿瘤边缘出现高灌注组发生转移的机会明显高于轻中度灌注组.②肿瘤边缘出现高灌注带组转移发生率明显高于无高灌注带组.③肿瘤边缘高灌注带与镜下血管计数呈正相关.结论 可以从肝细胞癌边缘有无异常血流灌注来判断肿瘤是否发生潜在转移.

Relation among tumor metastasis,prognostic and blood perfusion of hepatocellular carcinoma margin using CT Perfusion imaging

Objective To investigate the relationship between the hemodynamic changes and tumor metastasis of primary hepatocellular carcinoma (HCC), using CT perfusion imaging for evaluation the potentiality of tumor metastasis. Methods The study cohort included 34 patients confirmed HCC by operation and pathology.All cases were underwent CT double-phase dynamic scan and tumor margin perfusion CT study using a multi-slice CT. Perfusion parameters were measured for the HCCs,and compared those parameters with patients' follow-up information. hypertransfusion cases than those sligh or midrange ones in tumors' was positive correlation between hypertransfusion band in tumors' margin and the amount of the blood vessels by microscope.Conclusion We can judge the potentiality of tumor metastasis by the unnormal changes of primary hepatocellular cacinoma.
