文化程度对糖尿病控制状况的影响研究

目的了解本辖区内不同文化程度糖尿病患者疾病控制现状,为不同文化程度糖尿病患者的早期干预及治疗、护理决策提供依据。方法选择本辖区内病程1年以上的糖尿病患者600例,按文化程度分为＂初中及以下＂和＂高中及以上＂两组各300例。测量两组患者空腹血糖（FPG）、早餐后2h血糖（P2hPG）、糖化血红蛋白（HbA1c）、尿微量白蛋白（UAb）及眼底情况,并进行比较 使用糖尿病控制状况量表（CSSD-70）进行测评,比较不同文化程度患者的疾病控制情况差异。结果初中及以下的糖尿病患者FPG、P2hPG、HbA1c高于高中及以上文化程度患者（P〈0.05） 初中及以下文化程度糖尿病患者CSSD-70量表总分及生活习惯、生存技能、治疗目标、疾病知识维度得分均低于高中及以上文化程度患者（P〈0.05）。结论文化程度低的糖尿病患者疾病控制状况差于文化程度高的患者,因此要加强文化程度较低的糖尿病患者的治疗和管理水平,加强糖尿病健康教育,大力推广新的治疗策略,预防晚期并发症,提高生活质量,降低医疗费用。

Influence of Educational Levels on the Control of Diabetes

Objective The purpose of the present study was to find out how the sufferers from diabetes of educational levels in the area in order to provide evidence for early intervention and treatment as well as nursing.Methods 600 patients who had suffered from the disease for at least one year in the area were selected and divided into two groups according to their educational level evenly：（junior school and those below/Group Ⅰ,senior school and above Group Ⅱ）.For each group,FPG before breakfast,P2hPG after breakfast,HbA1c,Uab and the condition of the back of the eye were mesured and compared by using CSSD-70.Results FPG,P2hPG,and HbA1c of Group Ⅰ was higher than that of Group Ⅱ（P0.05）.The total score of CSSD-70 of the Group Ⅰ and the score of living habit,living capability,treating goal as well as knowledge of the disease were lower than that of Group Ⅱ（P0.05）.Conclusion The control of the disease for Group Ⅰ was lower than Group Ⅱ.So the treatment and management as well as the education of the disease to them should be enhenced.New treating strategy should be developed to prevent later complication and promote the living standard as well as keep down the cost of treatment.
