十二指肠损伤的诊断与治疗40例

目的探讨提高十二指肠损伤的早期诊断率和选择合理的手术方法。方法采用回顾分析40例十二指肠损伤病人的临床资料的方法。结果全组患者中，损伤位于十二指肠第一段12例，第二段与第三段之间24例，第四段4例，合并其他脏器伤4例；手术后并发症发生率25％，本组治愈率90％，病死率10％。结论注意十二指肠损伤的特点，早期诊断，早期掌握手术探查指征和选择合理的手术方案，术后有效的十二指肠减压，充分引流以及抗感染和支持治疗等，可提高治愈率。

Diagnosis and Treatment of 40 Cases with Duodenal Injuries

Objective To explore how to increase the early diagnosing rate, and to select the rational surgical treatment. Methods The data from 40 cases of duodenal injuries was retrospectively analyzed. Results In a total of all patients, the first part of the duodenum was injured in 12 cases, between the second and third part in 24 cases, the fourth part in 4 cases, and other complicating organic injuries in 4 cases; The postoperative incidence of complications, the curative rate and the morbidity of the total group were 25%, 90% and 10%, respectively. Conclusion The clinical characteristics of duodenal injuries should be grasped, early diagnosis must be paid closer attention, the early operative exploratory indications and rational selection of optimal surgical procedures should be mastered, postoperatively effective decompression of the duodenum, adequate drainage ,and anti-infection and supportive treatment should be oerformed .so as to raise the curative rate.
