摘    要:目的 研究肠黏膜屏障损害与内毒素血症的相互关系。方法 采用兔做动物模型,在常温无菌条件下机械性阻断肝十二指肠韧带造成肠道缺血、缺氧、肠黏膜屏障损害,肠道内大量内毒素穿过肠壁进入门静脉,造成内毒素血症。于对照组及实验组阻断前和阻断后不同时间,分别采血测血内毒素值,并作肠黏膜的病理检查。结果 肝门阻断后,门静脉血内毒素值明显高于阻断前和对照组( P < 0-01),肠黏膜病理改变也随时间延长,损害逐渐加重。结论 肠道缺血缺氧是肠黏膜屏障损害的直接原因,内毒素与肠黏膜损害形成恶性循环,加重屏障损害。预防和减少肠源性内毒素的产生对指导临床工作有利无弊。

Abstract:Objective To study the relationship between intestinal barrier damage and endotoxinemia. Methods Hepatoduodenal ligament was blocked under normal temperature and aseptic condition in rabbits modelto lead to an intestinal anoxia and damage of intestinal mucosa barrier, and then a great quantity of endotoxin would enter portal vein through intestinal wall resultingin endotoxinemia . Endotoxin level of portal vein blood was determined and pathology of intestinal mucosa was examined in control and experimental group ,including pre blockage and differenttimes of post blockage. Results Endotoxin level was significantly higher after blockage than that before blockage and control ( P < 0-01) . Pathological change of intestinal mucosa aggravated gradually with prolongation of blockage . Conclusion Intestinal anoxia is the direct cause of mucosa damage . Endotoxinemia and intestinal mucosa damage can form a vicious circle aggravating the pathologicallesion .Preventing and reducingtheforming ofendotoxinis beneficialto medical management.
