老年人Garden I型股骨颈骨折的保守治疗

目的 探讨老年GardenI型股骨颈骨折愈合的影响因家,寻求最佳保守治疗方法,以减少再移位风险。方法 对1994～2000年65岁以上34名患新鲜GardenI型股骨颈骨折的患者经保守治疗情况进行回顾。结果 22例(64．8％)在伤后6个月获愈合,12例(33．2％)未愈合,其中,受伤2周以后开始由床上训练过渡到轮椅训练,4周以后开始行走的患者,其愈合率相对高于在伤后2周内即开始过渡到轮椅训练。伤后4周以内即开始行走的患者,两组骨折愈合与不愈合患者股骨头外团或后倾程度无统计学差异。结论 对GardenI型股骨颈骨折的老年患者,卧床至少2周,2周以后开始由床上训练过渡到轮椅训练,4周后开始行走,可获得较好的保守治疗效果。

Analysis of Conservative Treatment on Garden Stage 1 Femoral Neck Fracture of Elderly People

Objective TO explore the factors that influence fracture union with fresh Garden stage I femoral neck fracture,identify the best conservative treatment method in order to miminize the risk of secondary displacement.Methods the conservative treatment of 34 patients aged 65 and elder with fresh Garden stage I femoral neck fracture between 1994 and 2000.were retro spectively studied.Result 22(64,8%) fractures united within 6 months after injury,12 (33.2%) failed to unite.the percentage of united fracture in patients who began bed-to-wheelchair transfer training after two weeks and began ambulation four weeks or later after injury was considerably higher than those patients who began bed-to-wheelchair transfer training within two weeks and began ambulation within four weeks.their were no statistical fifferences in the degree of valgus or retroversion of the femoral head between these two groups between fracture union and non-union. Conclusion To old patients with fresh Garden stage I femoral neck fracture,the best union rate can be expected when patients are confined to bed for 2 weeks,begin bed-to-wheelchair transfer training after then,and begin ambulation at 4 weeks or later after injury.
