原发性高血压患者脉搏波传导速度与靶器官损害的相关性研究

摘    要:目的探讨原发性高血压患者肱踝脉搏波传导速度(baPWV)与靶器官损害的关系。方法对所有患者行心电图、超声心动图、颈动脉超声和baPWV检测,并查尿微量蛋白。根据baPWV测量结果,将106例原发性高血压患者分为baPWV〉1400cm/s组(A组)61例,baPWV≤1400cm/s组(B组)45例。结果baPWV〉1400cm/s组靶器官损害发生率高于baPWV≤1400cm/s组(P〈0.05);颈动脉内膜厚度(CIMT)、颈动脉斑块评分(Crouse-s)、尿微量蛋白(mAlb)、左心室重量指数(LVMI)较baPWV≤1400cm/s组明显增高(P〈0.05);直线相关分析示baPWV与CIMT、LVMI成正相关(P〈0.05)。结论高血压患者心脑血管事件发生率明显增加与动脉弹性降低有关;靶器官损害与增加的baPWV密切相关。

Abstract:Objective To investigate the relationship between brachial - ankle artery pulse wave velocity and target organs damage in patients with essential hypertension. Methods Electrocardiography, echocardiography, carotid ultrasonography, brachial - ankle artery pulse wave velocity, microalbum inuria were measured in all patients. 106 essential hypertensive patients were divided into two groups according to the levels of baPWV : baPWV 〉 1400cm/s group ( group A, n = 61 ) ; baPWV ≤ 1400cm/s group ( group B, n = 45). Results Compared with subjects in group A and group B, group A showed higher the incidence rate of target organs damage (P 〈 0. 05 ) and higher levels of left ventricular mass index (LVMI), carotid intimamedial thickness (CIMT), carotid plaque score (Crouse -s) and microalbum inuria (mAlb) than that in group B (P 〈0.05). In linear correlation analysis, baPWV was positive correlation with CIMT, mAlb and LVMI (P 〈 0. 05). Conclusion The higher incidence rate of heart, brain and blood vessel events are related with artery elasticity decreasing; Target organs damage is close correlation with baPWV increaseing in patients with essential hypertension.
