针刺跷脉与穴位埋线治疗围绝经期失眠的疗效对比研究

目的:对针刺跷脉与穴位埋线治疗围绝经期失眠进行临床疗效评价。方法:选取60例围绝经期失眠患者,依据随机数字表法进行随机分组。针刺组30例,予针刺跷脉主穴及辨证配穴治疗;埋线组30例,予穴位埋线治疗。治疗4周后采用匹兹堡睡眠质量指数(PSQI)量表、kupermann评分量表进行评分,对比两组治疗前后雌二醇(E2)体内变化情况和临床疗效。结果:针刺组睡眠障碍、日间功能及PSQI总分优于埋线组,差异有统计学意义(P<0.05);针刺组kupermann评分除皮肤蚁走感、泌尿系感染两项治疗前后比较差异无统计学意义(P>0.05)外其余项目差异均有统计学意义(P<0.01),埋线组组内治疗前后比较除皮肤蚁走感差异无统计学意义(P>0.05)外,其余项目差异均有统计学意义(P<0.05);针刺组治疗后失眠、抑郁疑心及疲倦kupermann评分优于埋线组(P<0.05);针刺组E2升高更显著(P<0.05);埋线组总有效率83.33%(25/30),低于针刺组总有效率96.67(29/30),针刺组临床疗效更显著(P<0.05)。结论:针刺跷脉能有效改善围绝经期失眠,疗效优于埋线治疗,值得临床推广。

Comparative Study on the Therapeutic Effects of Needling Qiao Vessel with Acupoint Catgut Embedding in the Treatment of Perimenopausal Insomnia

Objective:To evaluate the clinical curative effect of needling Qiao Vessel and acupoint catgut embedding in the treatment of perimenopausal insomnia.Methods:A total of 60 cases met the inclusion criteria were randomly divided in the acupuncture group(n=30)and the control group(n=30).The control group was treated with acupoint catgut embedding,and the treatment group was treated with needling points of Qiao Vessel and other points according to syndrome differentiation.After 4 weeks of treatment,The scores of Pittsburgh sleep quality index(PSQI)and Kupermann,changes of estradiol(E2)before and after the treatment,as well as the clinical efficiency were observed and compared between the two groups.Results:The improvements of sleep disorder,daytime function and total score of PSQI were more significant in the acupuncture group than those in the control group(P<0.05).There were significant statistical differences in the Kupermann scores between before and after the treatment in the control group and in the acupuncture group(P<0.01);except for the scores in terms of skin ant walking sensation and urinary tract infection(P>0.05).Kupermann scores of insomnia,depression and fatigue were significantly improved in the acupuncture group than those in the control group(P<0.05).The increase of E2 in the acupuncture group was more significant than that in the control group(P<0.05).The total effective rate of the control group was 83.33%(25/30),which was significantly lower than 96.67%(29/30)of the acupuncture group(P<0.05).Conclusion:Needling Qiao Vessel can effectively improve perimenopausal insomnia,the effect is better than acupoint catgut embedding,which is worthy of clinical promotion.
