腹腔镜手术与开放性手术治疗胆总管结石疗效比较

目的：比较腹腔镜手术与开放性手术治疗胆总管结石的临床疗效及安全性。方法：选取2008年4月至2012年9月收治的100例胆总管结石手术患者作为研究对象,按照手术方法不同分为A组（腹腔镜治疗组）48例与B组（开放手术治疗组）52例,比较两组患者手术疗效及术后并发症情况。结果：两组患者手术均成功,无死亡病例。但A组患者手术时间、术中出血量、术后肛门排气时间及住院时间方面均显著少于B组（P〈0.01）,术后并发症发生率显著低于B组（X2=12.057,P〈0.01）,具有统计学意义。结论：腹腔镜手术治疗胆总管结石具有疗效佳、损伤小、术后并发症低、恢复快等优点,值得推广应用。

Comparison of Therapeutic Effect between Laparoscopic Operation and Open Operation in Treatment of Choledocholithiasis

Objective： To compare clinical efficacy and safety of laparoscopic operation and open operation in treatment of choledocholithiasis. Method： 100 cases of choledocholithiasis from 2008 April to 2012September were selected as the research object,according to the different operation ways,they were divided into group A（ laparoscopic group,48 cases） and group B（ open operation treatment group,52 cases）,operation efficacy and postoperative complications of two groups were compared. Result： The two groups of patients with operation were successful,there were no death cases. But operation time,intraoperative bleeding volume,postoperative anal exhaust time and hospital stay of the group A were significantly less than that of group B（ P＆lt;0.01）,the incidence of postoperative complications in group A was lower than that in group B（ X2= 12.057,P ＆lt;0.01）,There’s statistical significance. Conclusion： Laparoscopic operation for the treatment of choledocholithiasis has advantages of good curative effect,little injury,less postoperative complications and quick recovery,it is worthy of popularization and application.
