穿刺活检术在良恶性肿瘤诊断中的临床应用

目的：探讨经皮穿刺活检术在肝、肺、乳腺、淋巴结、甲状腺、卵巢等肿瘤诊断中的临床应用价值。方法：分析我科在超声、CT、C型臂DSA血管造影机等引导下或徒手触诊经皮穿刺活检术的病例资料111例。结果：本组共穿刺111例患者，112个部位，取得病理标本325个，活检阳性率为86．6％（97/112），假阴性率为13．4％（15/112）。病理诊断恶性肿瘤93例，良性病变8例。除CT引导下行肺肿物穿刺出现气胸或轻度肺损伤外，其余均未出现明显的并发症。结论：在超声、CT、C型臂DSA血管造影机等引导下或徒手触诊经皮穿刺活检术在良恶性肿瘤诊断中准确率较高且安全，具有较好的临床应用价值，值得推广应用。

Clinical Application of Puncture Biopsy in the Diagnosis of Benign Tumor and Malignant Tumor

Objective：To explore the application of percutaneous puncture biopsy in the diagnosis of liver, lung ,mammary glands, lymph nodes, thyroid gland and ovary twmer.Method：Analyzed the data of 111 percutaneous puncture biopsy cases which were conducted our department through monitored by ultra -sound, CT and digital subtraction angiography , or by palpation only.Result： 111 patients were punctured and 112 sites were punctured .325 piece of specimen were gained .Of all the sites were punctured , the positive rate of puncture was 86.6%（97/112）, false negative rate was 13.4%（15/112）.Of all 111 patients, 93 patients were with malignant tumor by pathological diagnosis , while 8 patients were benign lesion .Except for pneumothorax or mild injury of lungs , which happened in the puncture of lung goiter that were conducted by CT monitored, all patients had no transparent complication .Conclusion： Percutaneous puncture biopsy , which conducted through monitored by ultrasound , CT, digital subtraction angiography , or by palpation only, It is a way with high accuracy and safety in the diagnosis of benign tumor and malignant tumor .It possesses better value of clinical application , and it is worthy of generalization .
