先天性阴道畸形阴道成形术66例临床分析

目的:探讨阴道成形术患者的临床特点及术式选择。方法:回顾性分析66例先天性阴道畸形行阴道成形术患者的临床资料。结果:47例先天性无阴道患者中,8例伴周期性或不规则下腹痛(17.0%);19例阴道闭锁患者中16例伴下腹痛(84.2%)。乙状结肠法术后性生活满意率最高(75%),但手术时间和出血量均较多;改良的小阴唇皮瓣法术后外阴形态及创面恢复良好,性生活满意率高(66.7%)。结论:对合并下腹痛的先天性阴道畸形患者进行腹腔探查是十分必要的;对于小阴唇丰满者改良的小阴唇皮瓣法值得推广。

Clinically Characteristic Analysis of 66 Patients Undergoing Vaginoplasty with Congenital Vaginal Anomalies

Objective:To explore the clinical features and the therapeutic approach in women of congenital vaginal abnormity.Methods:A retrospective study was carried out on the clinical materials of 66 cases of vaginoplasty.Results:In 47 patients with congenital absence of the vagina,8 patients accompanied with cyclic or irregular pelvic pain(17.0%).16 of 19 patients with vaginal atresia accompanied with pelvic pain (84.2%).Patients treated with sigmoid loop for vaginoplasty reported most satisfactory coital function (75%) with longer operative time and more hemorrhage.Patients undergoing reformative labia minora flap vaginoplasty had good configuration of vulvae and higher satisfactory of coital function (66.7%) after surgery.Conclusions:It is necessary of intraabdominal exploration in patients with pelvic pain.Reformative labia minora flap vaginoplasty should be a basic consideration for the patients who have plumy labia minora.
