罗哌卡因单独或复合肾上腺素用于腋路臂丛麻醉的药代动力学研究

摘    要:目的了解罗哌卡因用于腋路臂丛麻醉的药代动力学特征,验证罗哌卡因与肾上腺素有无合用的必要。方法选取急诊做指动脉岛状瓣手术的16例患者,随机分为RP单用组(RP组)及其与肾上腺素合用组(RP+AD组),每组8例,均采用腋路臂丛麻醉,采用HPLC测定血浆罗哌卡因浓度。结果两组血药浓度均在相同的范围内,血药浓度峰值(Cmax)、达峰时间(Tmax)和血药浓度-时间曲线下面积(AUC)两组相比无统计学差异(P〉0.05)。结论罗哌卡因加或不加肾上腺素对血药水平没有影响。本研究中仅观察了注药后12小时内的血药浓度变化,因此不能完全表明罗哌卡因在腋路臂丛麻醉中的药代动力学参数。

Abstract:Objective This study is to understand the pharmacokinetics of ropivacaine and to confirm the necessity of ropivacaine with adrenaline for axillary brachial plexus anesthesia. Methods This study includes 16 emergent patients to finger artery island flap surgery under axillary brachial plexus anesthesia.They were randomly divided into two groups:Group RP(ropivacaine group,n=8) and Group RP AD(ropivacaine with adrenaline group,n=8). The total plasma concentration of RP was determined by high performance liquid chromatography. Results The study of the pharmacokinetics in anesthetized patients improves there was no significant difference between two groups(P>0.05).Conclusion There was no affect to the plasma concentration with or without adrenaline,only observing the concentration change in 12 hours,it can not indicate completely the pharmacokinetics parameters of ropivacaine for axillary brachial plexus anesthesia .
