微波消融结合三维适形放疗治疗原发性肝癌疗效观察

目的探讨微波消融治疗结合三维适形放疗治疗原发性肝癌的疗效及应用价值。方法选取44例原发性肝癌患者,分为对照组和实验组,对照组24例单纯行三维适形放疗,实验组20例放疗前行B超导向下经皮肝穿刺微波消融治疗。在治疗结束后进行疗效评价。结果实验组和对照组有效率分别为67.86%和40.00%;实验组和对照组的0.5、1、1.5、2年累积生存率分别为100%、91%、73%、49%和95%、50%、43%、35%;中位生存期分别为29个月和23个月;0.5、1、2年累积复发转移率分别为7.14%、14.29%、32.14%和9.09%、34.55%、65.45%。两组毒副反应相似,均未发生严重并发症。结论微波消融与三维适形放疗结合的方法治疗原发性肝癌并发症少,疗效好。

Microwave Coagulation Therapy Combined with Radiotherapy to Treat Primary hepatocellular carcinoma

Objective To investigate therapeutic effect and application value of microwave coagulation therapy combined with radiotherapy to Primary hepatocellular carcinoma.Methods 24 patients with Primary hepatocellular carcinoma were treated only with radiotherapy.20 patients were treated with the percutaneous microwave coagulation therapy guided by ultrasonic was done before of radiotherapy.After the treatment,the therapeutic efect was evaluated.Results The effect rateswere 67.86% and 40.00% in the therapy group and control group respectively(P<0.05).The median survival time,and the 0.5-,1-,1.5-,2-year cumulative survival rate were 29 months,100%,91%,72% and 49% in therapy group respectively,while were 23months,95%,50%,43% and 35% in control group respectively(P<0.05).The 0.5-,1-and 2-year cumulative recurrence andmetastasis ratewere 7.14%,14.29% and 32.14% in therapy group respectively,while were 9.09%,34.55% and 65.45% in controlgroup respectively(P<0.05).Conclusion Microwave coagulation therapy combined with radiotherapy to Primary hepatocellular carcinoma is a safe and effective treatment.
