小儿先天性胆总管囊肿合并严重感染的治疗体会

目的 寻找治疗小儿先天性胆总管囊肿合并严重感染的最佳治疗方法，以减少术后并发症，提高治愈率和生活质量。方法 对16例小儿先天性胆总管囊肿合并严重感染采取的不同治疗方法进行回顾性临床分析，探讨不同的治疗方式对其预后的重要性。结果 本组中9例先行囊肿外引流术，再行囊肿切除、胆道重建术，均治愈；3例I期囊肿切除、胆道重建术，其中2例死亡，1例反复出理胆管炎；4例行囊肿空肠吻合术，其中3例需要再次手术。结论 对于小儿先天性胆总管囊肿合并严重感染比较好的手术方法为先行囊肿外引流术，Ⅱ期再行囊肿切除、胆道重建术。

The experience of treatment for choledochal cyst associated with severe infection in children

Objective searching for the best treatment for choledochal cyst associated with severe infection in children so as to lower down complications after surgery, increase cure-rate and improve life quality.Methods 16 cases (of children suffering from congenital choledochal cyst associated with severe infection) were-frealed with different methods and the results were dinicnllg stndied.Results 9 cases who had external drainage first and then cystectomy and biliarypassage reconstruction, were all cured; among 3 cases who imderwent immediately cystectomy and biliary passage reconstruction two died and the other one suttered pom recurrent cholangitis after operatin; 3 of the remaining 4 cases who had cystojejunostomy first needed another operation.Conclusions the method for treating consenital from choledochal cyst associated with following cystectomy and biliary passage reconstruction.
