摘 要: | 目的:检测原发性肝癌(Primary hepatic carcinoma,PHC)患者血清CXCL13、IL-31 的表达情况,探讨其对原发性肝癌诊断治疗和预后评估的临床价值。方法:收集78 例PHC 患者和36 例健康体检者血清标本,采用酶联免疫吸附实验(ELISA)检测CXCL13、IL-31 的表达水平,统计软件分析与临床病理特征和实验室指标的关系,Spearman 相关分析检验与肝功能预后指标的相关性,ROC 曲线分析CXCL13、IL鄄31、AFP 对PHC 的诊断价值。结果:PHC 患者血清CXCL13 表达明显高于对照组(P<0.001),与肿瘤大小、转移、TNM 分期密切相关(P<0.05),与Child-pugh 评分(Child pugh score)呈正相关;CXCL13 联合AFP 的ROC 曲线下面积为0.938 ( P<0.05),灵敏度和特异性分别为82.8%、100%,较两种指标单独诊断时明显增高。然而,PHC 患者血清IL-31 的表达与对照组无显著差异,与临床各项指标均不相关。结论:CXCL13 在PHC 患者血清中高表达,其表达水平与肿瘤大小、转移、TNM 分期密切相关,并未发现IL-31 在PHC 患者血清中存在表达增高的现象。因此,CXCL13 可作为原发性肝癌诊断治疗及预后评估的血清学标志物。

Expression and significance of CXCL13 and IL-31 in serum of patients with primary hepatic carcinoma |

Abstract: | Objective:To investigate the expression of CXCL13,IL-31 in serum of patients with primary hepatic carcinoma (PHC) and analyze their clinical significance.Methods: ELISA was used to analyze the expression of CXCL13 and IL-31 in serum samples from 78 PHC patients and 36 healthy controls.The correlations of CXCL13,IL-31 expression with several clinicopathological in-dicators and laboratory indicators were statistically analyzed.The correlation coefficient was calculated by using the spearman correlation.Receiver-operating-characteristic (ROC) curves were used to determine the cutoff points yielding the highest specificity and sensitivity,and discriminative ability was evaluated by the area under the ROC curve.Results: Compared with the healthy controls,the level of CXCL13 were significantly higher in the serum of PHC patients ( P<0.001).The expression of CXCL13 in the serum of PHC patients was relative with the tumor size,the metastasis and the TMN stages (P <0.05).CXCL13 expression showed a positive correlation with child-pugh score.The AUC(Area under the curve)for using CXCL13 combined AFP levels to diagnose PHC was 0.938( P<0.05),sensitivity and specificity were82.8%,100%,respectively. Combined detection of CXCL13 and AFP was helpful for diagnosis of PHC.Nevertheless,IL-31 expression had no difference in PHC groups compared with Health controls.IL-31 expression was uncorrelated with clinicopathological indicators.Conclusion: CXCL13 is highly expressed in the serum of PHC patients.CXCL13 may be involved in the pathogenesis of PHC,which level has a closer relationship with tumor size,metastasis and TNM stage.CXCL13 can be an important indicator for the diagnosis and prognosis monitoring of PHC. |
