Correlation between aspirin resistance and platelet miR-223 in patients with acute cerebral infarction complicated with type 2 diabetes mellitus

Abstract:Objective To study the correlation between platelet miR-223 and aspirin resistance in patients with acute cerebral infarction complicated with type 2 diabetes mellitus.Methods A total of 120 patients with acute cerebral infarction in the First Affiliated Hospital of Hebei North University from December 2018 to December 2019 were retrospectively selected and divided into the combined group and the non-combined group according to the conditions of type 2 diabetes,with 60 patients in each group.The platelet indexes,maximum platelet aggregation rate(MAR),occurrence of aspirin resistance and level of miR-223 expression in the two groups were compared.The expression levels of miR-223 in aspirin resistant and aspirin sensitive patients were statistically analyzed.Results After treatment,the platelet hematocrit,mean platelet volume,platelet count and platelet distribution width in 2 groups were lower than before(P<0.05);before treatment,there were no significant differences in platelet hematocrit,mean platelet volume,platelet count,and platelet distribution width between 2 groups(P<0.05);after treatment,the platelet hematocrit,mean platelet volume,platelet count and platelet distribution width of patients in the combined group were(0.15±0.03)%,(7.33±1.22)fL,(121.22±9.22)×109/L,(15.33±2.44)%,which were lower than those in the uncombined group(0.21±0.11)%,(8.33±1.13)fL,(226.33±9.20)×109/L,(17.03±2.13)%],the differences were statistically significant(P<0.05).The MARS in the combined group were(76.88±6.74)%,(68.55±9.55)%,which were higher than those in the uncombined group(71.22±8.66)%,(51.22±9.22)%].When the inducers were adenosine diphosphate and chemotetraenic acid,the differences were statistically significant(P<0.05),the incidence of aspirin resistance was 33.33%,which was higher than that in the uncombined group(16.67%),the incidence of aspirin sensitivity was 66.67%,which was lower than that in the uncombined group(83.33%),and the level of miR-223 expression was 1.33±0.24,which was higher than that in the uncombined group(0.71±0.16),and the differences were statistically significant(P<0.05).The expression level of miR-223 in aspirin-resistant patients was 2.41±0.22,which was significantly higher than that in aspirin-sensitive group(0.98±0.17),and the difference was statistically significant(P<0.05).Conclusion The elevated expression of miR-223 on platelets in acute cerebral infarction patients with type 2 diabetes mellitus may be related to aspirin resistance.
