成人原发性甲状腺功能减退症临床诊断探讨

目的 :探讨成人原发性甲状腺功能减退症的临床特点。　 方法 :回顾分析 32例甲减病人临床表现及实验室数据。　结果 :临床表现无特异性 ,以粘液性水肿、疲乏、衰弱、贫血、胃肠及神经精神功能障碍为常见。青年人甲减起病较快 ,症状轻 ,甲状腺常可触及 ,伴粘液性水肿或贫血少见。中老年患者起病隐匿、病程长、症状复杂、贫血、粘液性水肿明显 ,甲状腺不肿大。一般S -TSH均明显增高 ,T3、T4 明显下降。　 结论 :年轻甲减病人症状轻 ,而中老年病人症状复杂 ,易误诊。

INVESTIGATION ON THE DIAGNOSIS OF HYPOTHYROIDISM

Objective To study the clinical characteristics of adult primary hypothyroidism Methods The clinical manifestations and laboratory examination results of 32 cases of hypothyroidism are analysed retrospectively. Results There has no speciality in the clinical manifestations. The main manifestations are myxoedema, fatigue, weak, anemia, gastroenteric and neuropsychic dysfunctions. To young patients, the onset of the hypothyroidism is fast, the symptoms are slight, the thyroids can usually be palpable, but the myxoedema and anemia are rarely complicated with. To middle age and senile patients, the clinical manifestations are just contrary to those of the young patients. Of the laboratory examination results, the levels of S-TSH increase and levels of T 3 & T 4 decrease remarkably. Conclusion The symptoms of hypothyroidism are slight in young patients and are complicated in middle age and senile patients, and it can easily cause misdiagnosis or missed diagnosis.
