改良Pfannenstiel切口在前列腺摘除术中的临床应用分析

目的：探讨改良Pfannenstiel切口在耻骨上经膀胱前列腺摘除术中的疗效和优势.方法：随机对63例BPH患者采用改良Pfannenstiel切口行耻骨上经膀胱前列腺摘除术,观察术后切口的愈合情况、手术平均时间,术后患者的体位、切口的疼痛、切口的张力、生活质量、术后并发症等指标的差异.结果：采用下腹部改良Pfannenstiel切口,完全可以替代直切口,切口暴露好,张力小,感染率明显下降,并发症少,切口小而美观,对合并腹股沟斜疝者无须另取切口,可一次完成.结论：改良Pfannenstiel切口在耻骨上经膀胱前列腺摘除术的治疗中是一种安全有效的方法,它具有操作简单.手术时间短,愈合速度快,出血量少,恢复快的优点,从解剖学上避免了下腹部腹直肌后鞘不完整造成的弊端,降低了切口疝的发生率;有很好的临床应用价值.

Modified Pfannenstiel Approach for Suprapubic Prostatectomy

Objective：To study the efficacy and investigate safety of Modified Pfannenstiel incision for suprapuhie prostatectomy. Methods： From May 2003to Dec 2006, 63patients with BPH underwent the treatments of Modified Pfannenstiel incision in suprapubic prostatectomy ,and the operative results were assessed using following parameters：mean operative time , blood loss, quality of life（QOL） heal of incision, tension of incison,bodily from of patient and operative complications. Results：Modified Pfannenstiel incision instead of a pure transverse incision,less complications, excellent exposure,minimal tension of incison, few infection rat, you needn, t take other incision as for patient complication incorporation hernia. Conclusions：The usage of Modified Pfannenstiel incision is an excellent treatment for BPH . It has mang advantages such as minimal injury , short operative time, few blood loss and rapid recoverment and provides both excellent exposure and better cosmetic results.
