无创正压通气治疗AECOPD的临床分析

目的探讨无创正压通气﹝NIPPV﹞治疗慢性阻塞性肺疾病急性加重期（AECOPD）合并Ⅱ型呼吸衰竭的疗效。方法选择60例AECOPD并Ⅱ型呼吸衰竭患者,随机分为对照组和治疗组,在常规治疗的基础上,对照组予持续低流量吸氧（2ml/min）,试验组予无创正压通气治疗,于治疗前,治疗24 h和5 d后分别记录动脉血气分析指标（PH,PaO2,PaCO2,BE,）呼吸频率,心率变化。.结果治疗组PH,PaO2,PaCO2,呼吸频率,心率变化优于对照组（P〈0.05）,BE变化高于对照组（P〈0.05）。结论无创正压通气治疗AECOPD并Ⅱ型呼吸衰竭疗效显著。

The clinical analysis of non-invasive positive pressure ventilation ( NIPPV ) in the treatment of AECOPD

Objective To study clinical efficacy of non-invasive positive pressure ventilation（NIPPV） in the treatment of acute exacerbation of chronic obstructive pulmonary disease（AECOPD） with type Ⅱrespiratory failure.Methods Sixty cases of AECOPD with type Ⅱ respiratory failure were randomly divided into control group and treatment group.The regular therapy together with inhale oxygen（2 ml/min） was given in the control group and regular therapy together with NIPPV in the treatment group.The level of arterial blood gas（pH,PaO2,PCO2,BE）,the respiratory rate（RR） and the heart rate（HR） were recorded retrospectively before and after 24 hours and 5 days in the two groups.Results pH,PaO2,PaCO2,HR,and RR were improved more in the treatment group than in the control group： P0.05.The base excess（BE） was higher in the treatment group than in the control group.Conclusion Use of NIPPV in treating patients with AECOPD complicated with type Ⅱ respiratory failure has good clinical effect.
