2种手术方式治疗胃十二指肠溃疡穿孔的临床观察及护理

目的比较损伤控制穿孔修补术和胃大部切除术治疗胃十二指肠溃疡穿孔的临床应用效果及护理。方法回顾性分析74例胃十二指肠溃疡穿孔患者的临床资料,其中应用损伤控制穿孔修补术54例(控制组)、行胃大部切除术20例(传统组)。比较2组患者围术期情况、住院天数、并发症发生率。结果控制组手术时间、排气时间、排便时间及住院天数均短于传统组,术中出血量少于传统组,并发症发生率低于传统组,差异均有统计学意义(P<0.05或P<0.01)。结论损伤控制穿孔修补术治疗效果优于胃大部切除术;做好术前术后护理、减少并发症是患者顺利康复的保证。

The clinical observation and nursing of two kinds of surgical in gastric ulcer perforations

Objective To investigate effect and nursing of damage control perforation neoplasty and stomach most resection in gastric ulcer perforations.Methods Retrospective analyzed the clinical data of 74 cases patients with gastric ulcer perforation,including 54 cases patients gave damage control perforation neoplasty（control group）,20 cases patients underwent stomach most resection（traditional group）.Compared with perioperative situation,hospital day,complication rate of two groups.Results The operative time,exhaust time,defecate time,hospital day of control group were shorter than those of traditional group,peri-operative bleeding was less than that of traditional group,complication rate was lower than that of traditional group,the difference were statistically significant（P0.05 or P0.01）.Conclusion Damage control perforation neoplasty in gastric ulcer perforations is better than stomach most resection;Complete the stomach preoperative postoperative nursing,reduce the complications of guarantee is the patient recover smoothly.
