新生儿泪囊炎的临床治疗方式及治疗时间的研究

目的研究新生儿泪囊炎的临床治疗方式及时机。方法选择2007年5月至2010年7月在广州医科大学附属深圳沙井医院眼科就诊的171例(202眼)新生儿泪囊炎患儿作为研究对象,根据年龄分为<1个月组、1³个月组、43个月组、4⁶个月组、76个月组、7¹2个月组以及112个月组以及1²岁组,分别为10例(15眼)、64例(75眼)、64例(72眼)、22例(27眼)、11例(13眼),依次给予手法按摩、泪道冲洗、泪道探通治疗,治疗1周后观察治疗后的泪道通畅情况。结果手法按摩后通畅眼数<泪道冲洗后通畅眼数<泪道探通后通畅眼数;年龄12岁组,分别为10例(15眼)、64例(75眼)、64例(72眼)、22例(27眼)、11例(13眼),依次给予手法按摩、泪道冲洗、泪道探通治疗,治疗1周后观察治疗后的泪道通畅情况。结果手法按摩后通畅眼数<泪道冲洗后通畅眼数<泪道探通后通畅眼数;年龄1³个月组的患儿治疗后通畅率和年龄43个月组的患儿治疗后通畅率和年龄4⁶个月组的患儿治疗后通畅率均为98.43%,通畅率显著地高于<1个月组、76个月组的患儿治疗后通畅率均为98.43%,通畅率显著地高于<1个月组、7¹2个月组以及112个月组以及1²岁组(P均<0.05)。结论新生儿泪囊炎治疗方式与治疗时间相关,<1个月的患儿建议采用手法按摩,12岁组(P均<0.05)。结论新生儿泪囊炎治疗方式与治疗时间相关,<1个月的患儿建议采用手法按摩,1⁶个月患儿可采用泪道冲洗、泪道探通治疗。

Study on the Treatment Method for Neonatal Dacryocystitis and the Choosing of Treatment for Neonatus with Different Ages

Objective To study the clinical effects of different ways in treating neonatal dacryocystitis and how to choose the way of treatment for neonates with different ages. Methods A total of 171 cases of neonatal daeryocystitis patients admitted in the department of ophthalmology, Shenzhen Shajing Hospital Affiliated to Guangzhou Medical University from May 2007 to Jul. 2010 were collected as the research object and divided into the 〈 1 month group（ 10 eases with 15 eyes）, 1-3 months group（64 eases with 75 eyes） ,4-6 months group（64 cases with 72 eyes） ,7-12 months group（22 cases with 27 eyes） and 1-2 years group （11 cases with 13 eyes） according to ages and all the patients were successively given chirismus, rinsing, channeling for treatment. The states of lacrimal duct patency 1 week after treatment were observed. Results Number of eyes with clear lacrimal gland after chirismus treatment 〈 number of eyes with clear lacrimal gland after rinsing treatment 〈 number of eyes with clear lacrimal gland after channeling treatment;patency rates of 1-3 months group and 4-6 months group were both 98.43% and significantly higher than 〈 1 month group,7-12 months group and 1-2 years group （ all P 〈 0.05 ）. Conclusion The way of treatment for neonatal dacryocystitis is associated with the age of neonatus, chirismus treatment is recommended for neonates 〈 1 month, rinsing, and channeling treatment is recommended for neonatus 1-6 months old.
