2型糖尿病肾病患者同型半胱氨酸和血流变学指标的临床意义

摘    要:目的观察同型半胱氨酸(HCY)和血流变学指标在2型糖尿病肾病患者中的变化,探讨其与糖尿病肾病的关系。方法根据尿白蛋白排泄率(UAER)将107例2型糖尿病患者分为单纯糖尿病(SDM)组和糖尿病肾病(DN)组,48名体检健康者作为正常对照组,分别测定HCY及血流变学指标。结果正常对照组HCY、全血粘度、血浆粘度及红细胞压积显著低于SDM组和DN组(P〈0.05)DN组HCY、全血粘度、血浆粘度及红细胞压积显著高于SDM组(P〈0.05)。HCY、全血粘度、血浆粘度及红细胞压积与UAER呈正相关(P〈0.05)。结论糖尿病患者普遍存在HCY、血液流变性异常,并随着UAER的增高而增高。检测糖尿病患者HCY、血流变学指标的变化,有助于预防2型糖尿病患者肾病的发生。

Abstract:Objective To observe the changes of plasma homocysteine (HCY) and hemorheological parameters in patients with type 2 diabetic nephropathy,and to study their relationships.Methods According to the result of UAER, 107 patients with type 2 diabetes mellitus were divided into simple diabetes mellitus (SDM) group and diabetic nephropathy (DN) group,and 48 healthy people were employed as controls.Plasma homocysteine and hemorheological parameters including whole blood viscosity,plasma viscosity and hematocrit were examined.Results There were significantly statistical difference of plasma homocysteine,the whole blood viscosity,plasma viscosity and hematocrit in SDM group and DN group as compared with those of control group(P〈0.05).Plasma homocysteine and the whole blood viscosity,plasma viscosity and hematocrit in DN group were significanttly highter than those in SDM group (P〈0.05). Plasma homocysteine and the whole blood viscosity,plasma viscosity and hematocrit were positively correlated with UAER (P〈0.05).Conclusions HCY and hemorrheology disorders were quite usual in diabetic patients,and these disorders were increased with the increment of UAER.Detecting the changes of HCY and hemorheological parameters in type 2 diabetes patients could anticipate the occur of diabetic nephropathy
