IVF-ET 助孕后妊娠分娩结局分析简

目的 研究体外受精、胚胎移植（IVF-ET）术后晚孕患者（≥28周）的妊娠分娩结局.方法 回顾分析2010年1月-2012年12月期间于安徽省立医院分娩的IVF-ET术后妊娠患者198例,其中单胎妊娠121例,双胎妊娠77例.比较两组的妊娠期并发症、分娩方式及产后出血.结果 IVF-ET双胎妊娠的早产率（72.7%）显著高于单胎妊娠（23.1%）（P＜0.01）,单、双胎妊娠重度子痫前期、前置胎盘、糖耐量（OGTT）异常、妊娠期肝内胆汁淤积综合征（ICP）重度、胎膜早破及产后出血发生率无明显差异（P＞0.05）.198例中双胎77例（38.9%）、胎膜早破34例（17.2%）,早产84例（42.4%）、产后出血17例（8.5%）.结论 IVF-ET晚孕患者胎膜早破、早产、产后出血均较自然受孕者明显升高,应加强孕期保健、及早发现、治疗妊娠期并发症,以改善妊娠结局.

Obstetric outcome of in vitro fertilization pregnancies

Objective To evaluate the perinatal outcome of pregnancy after in vitro fertilization and embtryo transfer （IVF-ET）. Methods Perinatal outcome of 198 pregnancies after IVF-ET was analyzed between Januat7 2010 to December 2012. Results IVF-ET twin pregnancy preterm birth rate （72.7%） was significantly higher than that of singleton pregnancies （23.1%） （P 〈0.01）. Single,twin pregnancy with severe pre-eclampsia, placenta previa,impaired glucose tolerance （OGTT） abnormalities,intrahepatic cholestasis syn- drome （ICP） with severe,premature rupture of membranes arid the incidence of postpartum hemorrhage had no significant differences （P 〉 0.05）. Of 198 cases of patients,there were twin 77 cases （38.9%）, premature rupture of membranes 34 cases （ 17.2% ） ,84 cases of preterm birth （42.4%） , arid postpartum hemorrhage 17 cases （8.5%）. Condusions The occurrences of preterm rupture of lnelnbrane,preterm deliver7 and postpartum hemorrhage were more in pregnancy after IVF-ET than in normal gestation. Paying attention to perinatal care and treating the complication in time are beneficial to pregnancy outcome.
