摘    要:目的 探讨三维超声成像在产科的临床应用价值。方法 采用表面成像、透明成像、多平面成像模式及经腹三维容积探头对部分中晚期妊娠的胎儿进行三维超声成像。结果 三维超声表面成像可直观反映胎儿体表结构的表面细微特征、立体形态及相互间的位置关系, 对胎儿面部、外生殖器、脐带、肢体等的评价有较高价值; 透明成像模式则能显示胎儿体内的骨性结构(肋骨、脊椎等)而不受其复杂曲率变化的影响, 成功率达100% ; 多平面成像模式则可对感兴趣区进行多角度的观察。结论 三维超声可作为二维超声的有益补充在产科发挥较大的作用, 可望提高对胎儿畸形的产前诊断率。

Abstract:Objective To evaluate the clinical application value of three dimensional ultrasonography(3DUS) in obstetrics. Methods Thirty pregnant women from 18 to 40 weeks of gestation were studied with transabdominal volume transducer, of which the frequency range was 3.0~5.0 MHz. The reconstruction methods included surface mode, transparent mode and multiplanar mode. Results Surface mode could clearly demonstrate the surface features of the fetus, such as the fetal face, fetal genitalia, umbilical cord, etc. Transparent mode could display the inner structures (mainly the bony structures) of the fetus, and their 3D images were not affected by their curvature. The successful rats was up to 100%. On the other hand, the region of interest could be viewed from various directions by using multiplanar mode. Conclusions 3DUS has special advantages and clinical value in obstetrics. It can serve as a beneficial supplement to two dimensional ultrasonography, but some work still should be done to further develop it.
