磁共振灌注成像在脑肿瘤中的初步应用

摘    要:目的 初步评价磁共振脑肿瘤灌注成像 (PWI)的成像方法及其临床应用价值。方法  5 3例脑肿瘤患者 ,根据平扫图像确定PWI层面 ,层面数为 6层 ,序列为GE -EPI ,成像时间 81s ,对比剂量为 0 .2mmol/kg ,注射速率 3ml/s ,PWI图像经工作站处理 ,计算并分析PWI图像和PWI参数 ,包括相对局部脑血流容积 (rrCBV)和相对平均通过时间 (rMTT)。结果 PWI的时间分辨率为 2s ,灰质的rrCBV平均为 2 .11,6例PWI图像存在几何变形 ,但未对灌注分析产生影响 ,3例鞍区肿瘤由于信号缺失致灌注失败 ,各种脑肿瘤的PWI参数有一定差异。结论 GE -EPI是PWI的适宜成像序列 ,但位于鞍区的小肿瘤PWI容易失败 ,初步的结果表明PWI有助于脑肿瘤的诊断

Abstract:Objective To evaluate methods of MR perfusion weighted imaging (PWI) in brain neoplasms and its clinical application value.Methods 53 patients with brain neoplasms underwent PWI, which was performed with GE EPI, injecting dosage 0.2 mmol/Kg, injecting rate 3 ml/s. Images were transferred into work station to create and analyze perfusion images and parameters.Results Time resolution was 2 s, the rrCBV of gray matter was 2.11. Because of signal defection, PWI were failuer in 3 patients whose neoplasm were located around sellae. The PWI parameters had some difference among brain neoplasms.Conclusion GE-EPI was suitable sequence to PWI, which had useful in diagnosis of brain neoplasms.
