老年维持性血液透析患者C反应蛋白与营养不良的关系

目的探讨老年维持性血液透析（MHD）患者C反应蛋白与营养不良的关系。方法选取老年MHD患者45例，采用主观综合营养评估法（SGA）评估营养状况，并检测透析前血C反应蛋白（CRP）、白蛋白（AIb）、前白蛋白（PA）、转铁蛋白（TF）及血红蛋白（Hb）。根据CRP水平分为正常组（CRP≤8mg／L）18例和升高组（CRP〉8mg／L）27例。结果CRP升高组营养不良的发生率（70.4％）显著高于CRP正常组（38．9％）（P〈0．05），CRP升高组血Alb、PA、TF和Hb水平均较CRP正常组明显降低妒〈0．05）。血CRP水平与Alb、PA、TF和Hb均呈显著负相关伊〈0．05）。结论老年MHD患者存在营养不良及炎症状态，炎症反应与营养不良密切相关。

Relationship between C-reactive protein and malnutriton in elderly maintenance hemodialysis patients

Objective To study the relationship between C-reactive protein and malnutriton in elderly maintenance hemodialysis （MHD） patients. Methods Subjective global assessment was used to evaluate the nutritional status in 45 MHD patients. Serum C-reactive protein （CRP）, albumin （Alb）, pre-albumin （PA）, transferrin （TF）, and hemoglobin （Hb） were measured. The patients were divided into normal CRP group （CRP≤8 mg/L） and elevated CRP group （CRP〉 8 mg/L）. Results Compared with normal CRP group, elevated CRP group had a higher incidence of malnutrition （70.4% vs 38.9%, P〈 0.05） and lower level ofAlb, PA, TF andHb （P 〈 0.05）. Moreover, the level of CRPwas negatively correlated with Alb, PA, TF and Hb, respectively. Conclusions Malnutrition and inflammation were very common among elderly MHD patients and the inflammatory reaction was closely related to malnutrition.
