| 摘 要： | 目的 探讨上尿路移行细胞癌(transitional cell carcinoma of the upper urinary tract,UUT-TCC)患者行静脉尿路造影(intravenous urography,IVU)时,显影欠佳(poor or nonvisualization of renal collecting system,PNV)的阳性率及其与肿瘤临床病理参数的关系.方法 回顾分析1990年1月～2005年1月因U-UT-TCC行根治性手术患者172例的临床病理资料.统计并比较术前尿脱落细胞学、行IVU时充盈缺损和PNV的阳性率;采用logistic回归分析模型探讨PNV是否和UUT-TCC临床病理参数之间存在相关性.结果 172例患者中,144例(83.7%)有肉眼血尿,12例(7.0%)有镜下血尿;46例(26.7%)尿脱落细胞学阳性,36例(20.9%)显示充盈缺损,105例(61.0%)显影欠佳.PNV和尿脱落细胞学阳性率比较及PNV和充盈缺损阳性率比较,差异均具有显著性(P值分别为0.031,0.001).单变量logistic回归分析表明PNV和肿瘤分期、分级及肿瘤大小显著相关(P值分别为0.028,0.031,0.006);多变量logistic回归分析表明PNV和肿瘤分期及肿瘤大小显著相关,(P值分别为0.042,0.014).结论 在排除泌尿系结石、感粢及畸形等病因后,IVU中如果出现PNV,应当高度怀疑UUT-TCC可能性;而且UUT-TCC患者出现PNV常常预示着肿瘤分期较高和体积较大. |
