南昌市西湖区乙肝疫苗母婴阻断效果监测分析

目的评价南昌市西湖区现行乙肝疫苗（HepB）免疫策略的母婴阻断效果。方法通过西湖区2001-2006年部分街道、乡镇社区卫生服务中心对母亲为乙肝病毒表面抗原（HBsAg）阳性的儿童进行HepB免疫后的血清学监测资料，分析疫苗保护水平。结果727名儿童中，HBsAg阳性率为2．5％。母亲（HBsAg、HBeAg]双阳性者HBsAg阳性率高于母亲单阳性者，抗-HBc阳性率为6．1％。抗-HBs阳性率为85．7％，抗体几何平均浓度（GMC）为90．4mlU／ml，且母亲单阳性抗一HBs阳性率高于母亲双阳性者。HepB保护率为92．2％（95％CI：90．3％～94．1％），其中对母亲单阳性者的保护率为93．4％（95％CI：92．3％～94．5％），对母亲双阳性者的保护率为89．3％（95％CI：87．2％～91．4％）。结论西湖区现行HepB免疫策略对母亲HBsAg阳性的儿童的免疫保护效果较理想。

The Analysis of Surveillance of Hepavax-B Interruption Effects Mother Infected Hepatitis B to Infant

Objective Estimated the strategy of preventing children infected Hepatitis B through maternal immunization at Xihu District, Nanchang city. Methods Analyzed the effects of strategy through the surveillance data of the children＇s serum from part of street and town community health service centers in Xihu District during 2001-2006, whoes morthers were HBsAg. Result The positive rate was 2.5% in 727 children. The rate of whose mothers were HBsAg and HBeAg was higher than that of single-positive. The rate of Anti-HBc positive was 6.1%, and the rate of Anti-HBs positive was 85.7%. Geometric mean concentration （GMC） of antibody was 90.4mIU/ml. The rate of that whose mothers were single positive was higher than that of double positive. The protection rate of HepB was 92.2% （95%CI： 90.3% - 94.1%）.The proetection rate of the mothers single-positive was 93.4% （95% CI： 92.3% - 94.5%） and that of double positive was 89.3% （95% CI： 87.2%-91.4%）. Conclusion The protection effect of the immunization strategy was satisfactory to the children mother was HBsAg positive was satisfactory .
