数字减影血管造影在颈动脉体瘤术前的应用

目的　探讨颈动脉体瘤术前数字减影血管造影 (DSA)的应用。方法　 7例颈动脉体瘤患者 ,行DSA瘤体造影 ,术前再次利用DSA行颈动脉体瘤滋养血管栓塞 ,并行健侧颈内动脉及颅内血管造影 ,观察大脑动脉环情况及血流侧支循环情况。结果　除颈动脉体瘤常见影像特征外 ,还发现另一种影像特征 :肿瘤将颈总动脉、颈内、外动脉推向前方 ,使颈动脉呈倒“Z”型 ,颈内、外动脉夹角变小。栓塞后可使术中出血量减少。颈内动脉及颅内血管造影 ,综合分析大脑动脉环情况及大脑侧支循环情况 ,可减少肿瘤手术切除的危险性。结论　DSA在颈动脉体瘤术前应用具有重要的临床价值

THE UTILIZATION OF DSA BEFORE OPERATION OF CAROTID BODY TUMOR

Objective To evaluate the effects of DSA before the operation of carotid body tumor. Methods 7 patients suffering from carotid body tumor were checked by DSA. Before tumor excision, tumors feeding vessels were embolized and images of the Willis circle and cerebral collateral circulation were observed by DSA. Results Besides the common images of carotid body tumors, another image was discovered that carotid artery was pushed ahead and showed a shape of “z” upside down, the angle between external and internal carotid artery became narrow. The tumors feeding arterial vessels embolization of 6 cases could be accomplished successfully before operation, the bleeding was decreased obviously in operation. By angiography to internal carotid artery and cerebral artery, the information of the Willis circle and cerebral collateral circulation was analyzed. So the danger of carotid body tumor excision was reduced. Conclusion The utilization of DSA before excision of carotid body tumor has important clinical values.
