恶性黑色素瘤免疫组化观察——附12例报告

目的： 探讨波形蛋白（Ｖｉｍｅｎｔｉｎ） 、Ｓ １００ 、ＨＭＢ４５ 、细胞角蛋白（Ｃｙｔｏｋｅｒａｔｉｎ） 的表达在恶性黑色素瘤（ 下称恶黑） 鉴别诊断中的意义。方法：１２ 例恶黑取石蜡切片各一张，用免疫组化链霉素 生物素（ＬＳＡＢ） 法，分别作Ｖｉ ｍｅｎｔｉｎ 、ＨＭＢ４５ 、Ｓ １００ 、Ｃｙｔｏｋｅｒａｔｉｎ 染色。结果： 每例Ｖｉｍｅｎｔｉｎ 、Ｓ １００ 、ＨＭＢ４５ 标记均呈阳性表达。Ｃｙｔｏｋｅｒａｔｉｎ 表达情况为，皮肤原发恶黑９ 例中有４ 例阳性，阳性者中２ 例为无色素性恶黑；３ 例淋巴结内转移性恶黑１ 例阳性。结论： Ｃｙｔｏｋｅｒａｔｉｎ 阳性不能排除恶黑的诊断

Expression of cytokeratin in malignant melanoma

Objective: Explore the significance of the expression of vimentin,S 100,HMB45 and cytokeratin in differential diagnosis of malignant melanoma.Methods: The paraffin sections of 12 malignant melanoma were stained by cytokeratin、vimentin、HMB45、S 100 using immunohistochemistry.Results: They were all positive with vimentin,HMB45 and S 100.The positive results of cytokeratin were 4/9 in the primary malignant melanomas of skin,2 of these 4 were amelanotic malignant melanomas,and 1/3 in the metastatic lymph nodes.Anti cytokeratin seemed to stain mostly and strongestly in the epithelioid cells.Conclusion: Cytokeratin reactivity,although it remains rare in paraffin sections,can no longer be said to rule out melanoma.
