立体放疗在70岁以上老年人肺癌治疗中的应用探讨

目的比较70岁以上老年人肺癌立体定向放疗和常规放疗的效果. 方法 1998年3月～1999年12月对70岁以上老年人肺癌26例(治疗组),采用以立体定向放疗为主加单药化疗(或不加)的综合治疗,与同期30例(对照组)采用常规放疗加化疗的70岁以上老年人肺癌比较.结果治疗组和对照组的1、2、3年生存率分别为69.23%、53.85%、38.46%和53.33%、36.67%、26.67%,差别不显著(P>0.05) ;但在相同带瘤生存的情况下,治疗组的生存质量明显提高(P<0.05).结论根据70岁以上老年人的身体、生理特点,采用以立体定向放疗为主加单药化疗(或不加)的综合治疗,既可保证患者的生存时间,又可提高其生存质量,符合70岁以上老年人肺癌患者的特点,值得临床进一步研究.

Clinical analysis of fractionated sterotactic radiotherapy for aged patients with lung cancer

Objective To compare the efficacy of sterotactic radiotherapy with that of conventional radiotherapy in aged patients with lung cancer.Methods From March,1998 to December,1999,26 cases over 70 year old patients with small solitary lesions in lungs received stereotactic radiotherapy and(or not) with single drug chemotherapy(the treatment group).The control group consisted of 29 cases of aged patients with lung cancer and received conventional radiotherapy after one to several cycles of chemotherapy.Results The 3 month local control rate was 91.18%.1 ,2 and 3 year survival rate of the two groups were 69.23%,53.95%, 38.46 % and 53.33%,36.67%,26.67%,respectively.No significant difference of survival rate between the two groups was found( P >0.05).The survival quality of patients bearing tumors in treatment group,however,was better than that in the control one( P < 0.05 ).Conclusions The results suggest that the stereotactic radiotherapy for aged patients with lung cancer is a good therapeutic selection with improved survival quality and identical survival rate.
