MRI在脑囊虫病中的诊断价值

目的探讨MRI在脑囊虫病诊断中的价值。方法回顾性分析海口市人民医院2011年11月—2017年6月经病理或临床确诊的27例脑囊虫病患者的临床和MRI影像学资料,其中MRI增强24例,分析不同临床分型脑囊虫病的MRI影像表现。结果根据MRI影像学特点,27例脑囊虫病患者分成4型,以脑实质型最常见,共17例(63%),脑室型4例(15%),蛛网膜下腔型和混合型各3例(11%)。脑囊虫病分3期,其中活体期11例,变性死亡早期6例,变性死亡晚期10例。活体期典型MRI影像显示病灶中存在等信号点状头节,病灶周围无水肿,增强无强化;变性死亡早期仍可存在头节,病灶周围出现轻中度水肿,增强出现环形强化;变性死亡晚期的MRI所见头节消失。结论 MRI可以为脑囊虫病的诊断提供重要的影像学依据,有助于脑囊虫病临床分期及治疗。

Diagnostic value of MRI for neurocysticercosis

Objective To evaluate the value of MRI in the diagnosis of neurocysticercosis.Methods The clinical and MRI imaging data of 27 patients with neurocysticercosis diagnosed by pathology or clinical follow-up from November 2011 to June 2017 were retrospectively analyzed,including 24 cases of MRI enhancement.Results According to the characteristics of MRI imaging,27 patients with neurocysticercosis were divided into four types,there were 17 cases (63％) of brain parenchyma for the most common type,4 cases of ventricle (15％),3 cases (11％) of subarachnoid type,and 3 cases (11％) of mixed type.Neurocysticercosis was divided into 3 stages,including 11 cases of living period,6 cases of degeneration and early death,10 cases of late degeneration.MRI showed that there was a nodular head in the lesion during the living period,with no edema and enhancement;in the early stage of degeneration,there was still a head section in the lesion,with mild to moderate edema around the lesion and ring enhancement.In the late stage of degeneration,the head section disappeared.Conclusion MRI can provide an important imaging basis for the diagnosis of neurocysticercosis,which contribute to the clinical stages and treatment of neurocysticercosis.
