糖尿病的社区护理干预

目的 探讨对社区糖尿病居民实施护理干预的效果.方法 调查并建立糖尿病居民的健康档案,制定糖尿病护理干预计划,大力进行健康宣教,建立良好的健康行为,定期体检、及早监护和治疗.结果 通过全方位的整体干预,现有糖尿病196人的病情已基本得到控制,管理随访率达100%,治疗达标率(血糖控制率)达95%.结论 实施社区护理干预,对不良生活方式引起的糖尿病进行早期预防比高科技的医疗手段更为有效.

Interference of Community Care to Diabetes

Object:To explore the interferential effect to diabetics living in community.Methods:lnvestigated and set up the health files of diabetics,a interferential plan for diabetics is formulated.In order to build up a healthful behavior,health education is provided to diabetics.Finally,all diabetics are examined regularly and are treated as early as well.Results:Through omnidirectional entire interference,the states of all diabetics(196)have been controlled fundamentally.The rate of following patients is 100 percent,and the rate of reaching the standard to treat the patients(reaching the standard of controlling blood sugar)is 95 percent.Conclusion:Through interference of community care and early preventing to the harmful life style which result easily in diabetes is better than high-technological treatment.
