超声弹性成像定性诊断甲状腺结节

摘    要:目的评价超声弹性成像技术定性诊断甲状腺结节的价值。方法对68例甲状腺结节患者的90个甲状腺结节进行常规超声和超声弹性成像检查,并进行硬度分级,计算病灶弹性成像图与灰阶声像图面积比值,将诊断结果与手术后病理结果进行对比。结果甲状腺恶性结节弹性成像硬度评分高于良性结节,恶性结节弹性成像图与灰阶声像图的面积比值高于良性病灶(P均〈0.05)。若以硬度分级≥4级、病灶弹性成像图与灰阶声像图面积比值≥1.48作为诊断恶性结节的标准,则弹性成像硬度分级联合面积比值诊断甲状腺恶性结节的敏感度、特异度、准确率分别为94.44%(17/18)、88.89%(64/72)、90.00%(81/90)。结论超声弹性成像硬度分级能判断甲状腺结节的相对硬度,结合结节弹性成像图与灰阶声像图面积比值有助于鉴别诊断甲状腺结节的良恶性。
