改良阴式子宫切除术108例分析

目的探讨非脱垂子宫经改良阴式子宫全切除术的临床应用价值。方法将2007年12月～2009年10月108例因子宫良性病变且为非脱垂子宫的患者采用改良阴式子宫切除术，对照组100例采用传统开腹子宫全切术。观察两组平均手术时间、术中平均出血量、肛门排气时间、下床活动时间、术后平均住院天数。结果观察组平均手术时间（63．00±35．28）min，术中平均出血量（93．00±50．18）mL，下床活动时间（24．2±9．08）h，肛门排气时间（28．16±11．72）h，术后平均住院天数（5．50±1．50）d；对照组平均手术时间（92．35±26．43）min，术中平均出血量（132．35±56．33）mL，下床活动时间（48．83±12．12）h，肛门排气时间（48．12±13．68）h，术后平均住院天数（7．4±2．6）d。两组比较有显著差异（P〈0．01）。结论对非脱垂良性子宫病变者实施改良阴式子宫切除术，手术时间短、出血量少、腹部无瘢痕、住院天数少。通过对手术方式的改进，使手术更安全、迅速，值得临床推广。

Improved Transvaginal Hysterectomy:A Clinical Analysis of 108 Cases

Objective To evaluate the clinical value of improved transvaginal hysterectomy for no-prolapse uterus. Methods All 108 cases of no-prolapse uterus disease treated by improved transvaginal hysterectomy from Dec. 2007 to Oct 2009 were analyzed as study group, and 100 cases were treated by transabdominal hysterectomy as control group. The operative time, blood loss,exhaust time, action time of getting up of bed and mean postoperative hospital stay of both groups were observed. Results The operating time was （63.00 ± 35.28）min, the blood loss was （93.00 ± 50.18）mL,the action time of getting up of bed was （24.2 ±9.08）h,the exhaust time was （28.16 ±11.72）h,and the mean postoperative hospital stay was（5.50 ±1.50）days in the study group, and those of the control group were（92.35 ± 26.43 ）min, （132.35 ±56.33 ） mL, （36.25 ±12.12）h, （34.12 ±13.68 ）h , and （7.40 ±2.60）d,with a significant difference between the two groups （P〈0.01）. Conclusion Transvaginal hysterectomy for no-prolapse uterus disease has remarkable advantages of less blood loss,reduced operating time,quicker recovery,less pain and no abdominal scar,and it is a safe and adopted operation.
