食管内支架在治疗食管贲门狭窄及食管瘘中的临床应用

目的探讨食管内支架在治疗食管、贲门狭窄,食管瘘中的疗效和术中术后的并发症及处理。方法根据病变情况选择支架,内镜下置入支架,观察并发症出现情况并及时处理。结果 146例病人中,126例一次成功置放支架,位置准确;6例位置偏低,使用支架提取器调整到正确位置;3例取出重放;11例不能常规置入导丝.经积极处理后9例置入支架,2例导丝未置入而放弃。10例支架术后再狭窄或又出现食管瘘,8例选择再置入支架。总共置入152个支架。随访中,5例支架下移,1例支架排出体外,3例因大量呕血死亡;生存期最短2个月,最长4年2个月。结论内镜下置入覆膜食管内支架操作简便,疗效好,但应重视严重并发症的预防与处理。

Clinical application of esophageal stent on esophageal and cardiac Stenosis

Objective To investigate the efficacy of the esophageal stent in curing esophageal and cardiac stenosis.Method To select the stent according to the cases of lesions,and then put it under the endoscopy to observe the complications and to deal with them as soon as possible.Result Of 146 patients,126 cases were implanted successfully in the exact place,6 cases with a little lower,and adjusted to the exact place with the help of the stent extractor and 3 cases removed and implanted once again.In the follow-up,5 stents shift down;1 stent eliminated from the body;3 cases died of profuse haemorrhage.Their survival durations vary from two months to four years and two months.Conclusion To implant the esophageal stent is easy to operate and has a good efficacy.
