| 摘 要： | 目的 探讨特发性血小板减少性紫癜患者进行腹腔镜脾切除术的方法和治疗效果。方法 对11例特发性血小板减少性紫瘢患者进行经腹腔镜脾切除术。结果 11例均顺利完成手术，处理脾周韧带时分别使用Ligasure 2例，超声刀8例，电刀1例；处理脾蒂应用Endo-GIA 9例．结扎及钛夹夹闭2例。术中脾窝均放置引流管，手术时间120～240min．出血10～200mL．平均术后5天痊愈出院。结论 原发性血小板减少性紫癜患者进行绎腹腔镜脾切除术安全且效果良好。

| Abstract: | ObjectiveTo evaluate the usefulness and therapeutic effect of laparoscopic splenectomy in patients of idiopathic thrombocytopenic purpura (ITP).MethodsLaparoscpic splenectomy was proformed on 11 patients with idiopathic thrombocytopenic purpura. ResultsAll the laparoscpic splenectomy operations were successful in the 11 cases. Peri-splenic ligament was dissected with Ligasure,Ultrasonic shears and monopole-electrocoagulation in 2 cases, 8 cases and 1 case respectively. Endo-GIA was used for the dissection splenic pedicels in 9 cases,while the other 2 cases were treated with ligation and titanium clipping. The operative time lasted for 120 to 240 minutes and the blood loss was 10-200 mL.The mean postsurgical stay was 5 days. ConclusionLaparoscopic splenectomy can be safely performed in patients of idiopathic thrombocytopenic purpura and the therapeutic effect is well. |
