鼻牙槽骨塑形对单侧完全性唇腭裂患儿术后鼻外形的影响

目的：探讨术前鼻牙槽骨塑形矫治对单侧完全性唇腭裂患儿术后长期鼻外形美观与对称性的影响。方法：84例患儿按改良式旋转推进唇裂修复术实施手术。其中，经过术前鼻牙槽骨塑形矫治42例，未经术前鼻牙槽骨塑形矫治42例。均采用术后4～5a照片打分方式进行鼻外形评定，而后分组进行比较。采用SPSS10．0软件包进行配对样本t检验。结果：经过术前鼻牙槽骨塑形矫治和未经术前鼻牙槽骨塑形矫治患儿，术后4～5a鼻外形的美观与对称性平均得分分别为66．62±14．25和66．31±15．08，两者之间无显著统计学差异（P〉0．05）。结论：单纯术前应用鼻牙槽骨塑形矫治纠正单侧完全性唇腭裂患儿鼻畸形，而未对单侧唇裂鼻畸形形成的解剖学机制进行有效干预，手术后良好的鼻外形无法长期维持。

Nasal symmetry in unilateral complete cleft lip/palate after primary cheiloplasty :Comparison of nasoalveolar molding and non nasoalveolar molding

PURPOSE： To assess the efficacy of presurgical nasoalveolar molding on long-term nasal symmetry and shape in unilateral complete cleft lip/palate after primary cheiloplasty. METHODS： Eighty four complete unilateral cleft lip/ palate patients were divided into 2 groups： presurgical nasoalveolar molding group （42 patients） and non presurgical nasoalveolar molding group（42 patients）. The primary cheiloplasty was conducted by the same surgeon. Photographs were taken and evaluated 4 to 5 years postoperatively. The data was analyzed with SPSS 10.0 software package for paired t test. RESULTS： The scores of the two groups were 66.62±14.25 and 66.31±15.08 respectively. There was no significant difference between the two groups （P〉0.05）. CONCLUSION： Nasoalveolar molding is an adjunctive therapy for facilitating primary cheiloplasty while it is not an effective treatment for nasal asymmetry in cleft patients.
